OK. I haven't been able to reproduce it at will yet... and we only see it occasionally in production. When I get more info I'll let you know. Likely to be awhile. :-(

I can only advice you to try to repeat the problem when running a debugger and
then single-step through the code until you see why it "returns silently".
Unless you can produce a test case that we can use to repeat this problem...
